Scheduling A Patient
There are two ways for referring practices throughout Arizona to schedule patients with Valley's interventional pain management specialists:
- Provide your patient with our contact information or Web site address, ValleyPain.org, to schedule an appointment at one of our 6 Valley-wide locations
- Provide our office with the patient's contact information and we will contact the patient to schedule an appointment
For our referring surgical practices, if your surgeon desires to order a specific procedure, please use the Procedure Specific Referral Form.
In order to ensure your patient has the best possible initial visit with one of our board-certified pain management specialists, we request the following information be sent to the appropriate office via fax prior to the appointment being scheduled:
- A Valley Pain Consultants Referral Form, including:
- Patient's current contact information
- Patient's insurance information
- Any records the patient may have from your office or a previous pain physician, including any past imaging records that may be applicable
- Insurance authorization if required
When completing the New Patient Referral Form, please choose the Valley Pain Consultants location to which you would like to refer to. If this changes once we are in contact with the patient, our office will take care of the transfer of medical information.
Please note that you may also choose the physician you would like to refer to or allow us to match the patient according to your patient's specific needs and physician availability.
Please ask your patients to bring the completed New Patient Packet with them to their initial visit to Valley Pain Consultants.
